Waiver of Liability, Assumption of Risk, Media Release, and Indemnity Agreement

In order to participate in any activities held by Shlack and Dance you must sign the liability waiver. By signing the waiver you hereby agree to release, waive, discharge, and covenant not to sue Shlack and Dance, its proprietors, employees, agents, orthe proprietors of the premises and its agents from liability from any and all claims including the negligence of Shlack and Dance resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in activities, classes, observation, and use of the facilities, premises, or equipment.
You acknowledge that dancing is a physical activity and that any physical activity inherently carries risks of injury that cannot be avoided regardless of the precautions taken to avoid them. Some dancing puts strenuous exertion on various muscle groups and the cardiovascular system. Risks include, but are not limited to, muscle soreness, falls, bruises, pulled muscles, concussions, broken limbs, paralysis, and death. You hereby assert that your participation is voluntary and knowingly assume all risks.
You agree to allow Shlack and Dance to collect photography, video, and other media of your person during any activities held. You waive any right to royalties or other compensation arising from or related to the use of the media and the right to approve or inspect the media.
You agree to INDEMNIFY AND HOLD Shlack and Dance harmless from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorneys’ fees brought as a result of your involvement at Shlack and Dance and to reimburse them for any such expenses incurred.
You had read this waiver of liability, assumption of risk, and indemnity agreement, fully understood its terms, and understand that you are giving up substantial rights, including your right to sue. You acknowledge that you are you signing this waiver freely and voluntarily, and intend your submission to be a complete and unconditional release of all liability to the greatest extent allowed by law.
